Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized case plans to support clients in achieving their personal and financial goals.
- Provide crisis intervention and emotional support to individuals and families facing challenging circumstances.
- Advocate for clients within social service systems to ensure access to necessary resources, entitlements, and housing.
- Maintain accurate, confidential, and timely case documentation in compliance with agency policies and legal standards.
- Facilitate support groups and skill-building workshops to promote community engagement and resilience.
- Collaborate with multidisciplinary teams (healthcare, legal, education) to coordinate holistic care for complex cases.
Qualifications
- Master of Social Work (MSW) degree from an accredited university, or Bachelor’s degree with relevant experience.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum of 2-3 years of experience in social work or case management within a community health or social services setting.
- Strong understanding of the local social service landscape and community resources.
- Excellent interpersonal and communication skills, with the ability to build trust with diverse populations.
- Ability to work independently and manage a high caseload effectively under pressure.
