Responsibilities
- Develop and implement comprehensive, individualized case plans for at-risk youth and their families.
- Conduct regular home visits and community assessments to evaluate safety and progress.
- Provide crisis intervention and conflict resolution support during high-stress situations.
- Coordinate with school systems, healthcare providers, and legal entities to ensure holistic service delivery.
- Maintain precise, HIPAA-compliant documentation of all client interactions and service outcomes.
- Facilitate weekly support groups and parenting workshops to promote family stability.
- Advocate for client needs within the state and local social service infrastructure.
Qualifications
- Master’s Degree in Social Work (MSW), Psychology, or a related behavioral science field.
- Valid LCSW or LSW licensure in the State of Illinois.
- Minimum of 3 years of experience in youth or family-centered case management.
- Proficiency in trauma-informed care practices and evidence-based interventions.
- Exceptional verbal and written communication skills with a focus on empathy.
- Valid driver’s license and reliable personal transportation for community outreach.
- Proven ability to manage a diverse caseload while meeting strict reporting deadlines.
