Responsibilities
- Assess client needs through comprehensive interviews and home visits to develop personalized care plans.
- Facilitate access to essential community resources, including housing, healthcare, and vocational training.
- Maintain precise, confidential electronic case records in accordance with state and federal regulations.
- Coordinate with cross-functional teams, including mental health professionals and legal advocates, to ensure holistic support.
- Advocate for client rights and interests within local government and community health agencies.
- Participate in crisis intervention efforts and provide immediate de-escalation support when necessary.
- Monitor and evaluate the progress of client goals, adjusting service plans as required for optimal outcomes.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related field required.
- Current LCSW or LSW licensure in the State of Illinois.
- Minimum of 3 years of experience in social casework or human services.
- Demonstrated expertise in crisis intervention and trauma-informed care practices.
- Exceptional documentation skills and proficiency in case management software.
- Deep understanding of the socioeconomic challenges facing urban populations in Chicago.
- Valid driver’s license and reliable transportation for community field visits.
