Responsibilities
- Conduct comprehensive psychosocial assessments to identify client needs, strengths, and barriers to success.
- Develop and implement individualized care plans in collaboration with clients, families, and interdisciplinary teams.
- Provide direct counseling and crisis intervention services to address mental health and substance use challenges.
- Advocate for clients to ensure access to essential resources, including housing, healthcare, and financial assistance.
- Maintain accurate and confidential case records in compliance with HIPAA and agency policies.
- Facilitate support groups and community outreach initiatives to promote client empowerment.
- Monitor client progress and adjust treatment strategies as needed to ensure positive outcomes.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution required.
- Valid state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW).
- Minimum of 2 years of experience in clinical social work or case management.
- Strong knowledge of community resources and social service systems.
- Excellent verbal and written communication skills.
- Empathy, cultural competence, and the ability to work with diverse populations.
- Proficiency in Microsoft Office Suite and case management software.
