Responsibilities
- Assess client needs through comprehensive interviews and evaluations
- Develop and implement individualized care plans with measurable goals
- Provide crisis intervention and ongoing counseling support
- Coordinate with healthcare providers, legal services, and community organizations
- Advocate for client rights and navigate complex social systems
- Lead outreach programs to identify and engage underserved populations
- Maintain detailed case documentation and comply with regulatory standards
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in Illinois (or ability to obtain within 3 months)
- Minimum 5 years of direct client experience in social services
- Expertise in trauma-informed care and crisis intervention
- Strong case management and documentation skills
- Experience working with diverse populations and cultural competency
- Valid driver's license and reliable transportation
- Proficiency in electronic health record systems
