Responsibilities
- Develop and implement individualized case management plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis interventions in emergency and community settings
- Collaborate with healthcare providers, legal advocates, and government agencies to coordinate holistic support services
- Lead trauma-informed support groups and community workshops on financial literacy and parenting skills
- Maintain detailed electronic records ensuring compliance with HIPAA and state regulations
- Supervise junior staff and social work interns, providing mentorship and performance feedback
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as a Licensed Clinical Social Worker (LCSW) in Illinois
- Minimum 5 years' experience in direct client services within social services or healthcare settings
- Proven expertise in crisis intervention and trauma-informed care methodologies
- Advanced proficiency in electronic health records (EHR) systems and documentation standards
- Spanish language proficiency preferred to serve diverse Chicago communities
- Valid Illinois driver's license and reliable transportation for home visits
