Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans tailored to client needs.
- Provide crisis intervention and emotional support to individuals and families facing acute challenges.
- Advocate for client rights and ensure access to necessary community resources and services.
- Collaborate with multidisciplinary teams, including healthcare providers and legal professionals, to ensure holistic care.
- Maintain accurate, confidential case documentation in accordance with agency standards and legislation.
- Facilitate group therapy sessions and workshops focused on mental health and wellness.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution.
- Valid registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum of 3 years of experience in social work or case management within a community or healthcare setting.
- Strong knowledge of Ontario’s Mental Health Act and relevant human rights legislation.
- Excellent verbal and written communication skills, with the ability to build rapport quickly.
- Bilingualism (French/English) is considered an asset.
