Key Responsibilities:
- Conduct comprehensive psychosocial assessments and develop individualized, strength-based care plans for clients.
- Provide direct counseling and crisis intervention services to individuals and families facing mental health, housing, or substance use challenges.
- Advocate for clients to ensure access to essential healthcare, legal, and social services.
- Collaborate with multidisciplinary teams, including medical providers and legal aid, to coordinate holistic treatment strategies.
- Maintain accurate and confidential client records in accordance with HIPAA and agency protocols.
- Facilitate support groups and educational workshops to promote community resilience and well-being.
- Monitor client progress and adjust care plans as necessary to ensure positive outcomes.
Qualifications:
- Master of Social Work (MSW) degree from an accredited university.
- Active Washington State Social Work License (LICSW) or LSW with eligibility for licensure.
- Minimum of 2-3 years of experience in case management, mental health, or social services.
- Strong knowledge of community resources and social service systems.
- Excellent verbal and written communication skills.
- Demonstrated ability to handle sensitive information with the highest level of confidentiality.
- Valid Driver’s License and reliable transportation (for community outreach).
