Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans in accordance with agency standards.
- Provide direct case management support, including advocacy, crisis intervention, and emotional support.
- Connect clients with critical community resources, such as housing, healthcare, employment services, and legal aid.
- Monitor client progress and adjust care plans as necessary to ensure positive outcomes.
- Collaborate with interdisciplinary teams, including medical staff and therapists, to ensure holistic care.
- Maintain accurate and confidential case records using our electronic health record system.
- Participate in community outreach events to raise awareness about available services.
Qualifications
- Master’s degree in Social Work (MSW) preferred; Bachelor’s degree with relevant experience considered.
- Valid New York State Social Work license or eligibility for licensure.
- Minimum of 2-3 years of experience in social services, case management, or a related field.
- Strong understanding of community resources and social service systems.
- Excellent written and verbal communication skills.
- Demonstrated ability to handle sensitive information with the utmost confidentiality.
- Valid driver’s license and access to a vehicle (required for client home visits).
