Responsibilities
- Conduct Comprehensive Assessments: Perform thorough evaluations of client needs, strengths, and challenges to develop accurate service plans.
- Develop Individualized Plans: Create and implement personalized support strategies to help clients achieve their short-term and long-term goals.
- Advocate & Resource Navigation: Act as a fierce advocate for clients, connecting them with housing, healthcare, employment, and financial assistance programs.
- Crisis Intervention: Provide immediate support and guidance during high-stress situations or mental health crises.
- Documentation: Maintain detailed, confidential, and timely case notes and reports in accordance with agency standards and legal requirements.
- Collaboration: Work closely with multidisciplinary teams, healthcare providers, and government agencies to ensure holistic client care.
Qualifications
- Education: Bachelor’s degree in Social Work (BSW) or a related field (e.g., Psychology, Sociology). Master’s degree (MSW) preferred.
- Licensure: Valid registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Experience: Minimum of 2-3 years of experience in social services, case management, or a related non-profit sector.
- Skills: Strong knowledge of community resources, funding streams, and relevant legislation (e.g., HST Act, Housing Act).
- Communication: Exceptional verbal and written communication skills with the ability to explain complex concepts to diverse audiences.
- Empathy & Resilience: Demonstrated ability to handle emotionally charged situations with professionalism and self-care.
