Responsibilities
- Develop and implement individualized case management plans for at-risk populations
- Provide crisis intervention and trauma-informed counseling to individuals and families
- Coordinate multidisciplinary care teams including healthcare providers, educators, and legal advocates
- Conduct community outreach to identify service gaps and program opportunities
- Train staff in trauma-informed practices and cultural competency
- Analyze client outcomes to refine program effectiveness and reporting
- Advocate for policy changes at local and state levels affecting underserved communities
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW or LICSW licensure in Massachusetts
- 5+ years of direct service experience in social services
- Expertise in crisis intervention and trauma-informed care models
- Experience supervising multidisciplinary teams
- Strong data analysis skills for outcome measurement
- Bilingual proficiency in Spanish or Haitian Creole preferred
- Valid Massachusetts driver's license with reliable transportation
