Responsibilities
- Client Assessment & Planning: Conduct comprehensive biopsychosocial assessments and develop individualized care plans tailored to client needs and goals.
- Crisis Intervention: Provide immediate support and intervention during mental health crises, ensuring client safety and stabilization.
- Case Management: Coordinate with medical providers, legal aid, housing authorities, and other community resources to facilitate client access to essential services.
- Counseling: Deliver individual and group counseling sessions utilizing evidence-based therapeutic modalities (e.g., CBT, DBT).
- Advocacy: Serve as a strong advocate for client rights and needs within healthcare and social systems.
- Documentation: Maintain accurate, timely, and confidential electronic health records (EHR) in compliance with HIPAA regulations.
- Program Development: Contribute to the continuous improvement of our social service programs through data analysis and feedback.
Qualifications
- Licensure: Master’s degree in Social Work (MSW) required; LCSW (Licensed Clinical Social Worker) or LMSW (Licensed Master Social Worker) license in New York State is mandatory.
- Experience: Minimum of 3-5 years of experience in clinical social work, case management, or mental health services.
- Communication: Exceptional verbal and written communication skills, with the ability to explain complex concepts to diverse audiences.
- Empathy: Demonstrated ability to build rapport and establish trust with vulnerable populations.
- Computer Proficiency: Advanced proficiency with electronic health records and Microsoft Office Suite (Excel, Outlook, Word).
- Bilingual: Bilingual (English/Spanish) is a plus but not required.
