Responsibilities
- Oversee and design comprehensive individualized service plans for high-needs urban populations.
- Provide clinical mentorship and daily operational guidance to a team of junior case managers and outreach specialists.
- Lead emergency crisis interventions and stabilize high-risk situations using trauma-informed methodologies.
- Forge strategic partnerships with healthcare providers, legal entities, and municipal agencies to streamline client care.
- Audit and maintain rigorous case documentation in alignment with state regulatory and grant-funding requirements.
- Advocate for policy changes and service improvements based on localized community data and client feedback.
Qualifications
- Master’s Degree in Social Work (MSW) from an accredited university program.
- Active New York State licensure (LCSW or LMSW) in good standing.
- Minimum of 5 years of post-graduate experience in case management or community-based social services.
- Demonstrated expertise in trauma-informed care and harm reduction principles.
- Superior interpersonal and de-escalation skills tailored for diverse socio-economic backgrounds.
- Advanced proficiency in electronic health record (EHR) systems and case management software.
- Fluency in a second language (Spanish or Mandarin) is significantly preferred.
