Responsibilities
- Provide crisis intervention and short-term counseling to individuals and families in acute distress
- Develop and implement personalized care plans connecting clients with housing, healthcare, and social services
- Collaborate with law enforcement, hospitals, and community organizations to coordinate emergency responses
- Conduct psychosocial assessments and risk evaluations for high-need populations
- Train staff in de-escalation techniques and trauma-informed practices
- Advocate for systemic changes through policy recommendations and community partnerships
- Maintain accurate case documentation and comply with HIPAA regulations
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW (Licensed Clinical Social Worker) certification in Illinois
- Minimum 3 years crisis intervention experience with vulnerable populations
- Certification in Trauma-Focused Cognitive Behavioral Therapy preferred
- Experience with electronic health records (EHR) systems
- Valid Illinois driver's license with clean driving record
- Bilingual in English/Spanish or other community languages
- Knowledge of Chicago's social services network and resources
