Responsibilities
- Provide crisis counseling and immediate intervention for individuals/families experiencing trauma, homelessness, or domestic violence
- Develop and implement personalized case management plans with measurable outcomes
- Collaborate with healthcare providers, law enforcement, and community organizations to coordinate resources
- Conduct psychosocial assessments and create detailed intervention strategies
- Lead community workshops on mental health awareness and crisis prevention
- Maintain accurate documentation while maintaining strict confidentiality standards
- Mentor junior staff and supervise practicum students
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW/LSW license in Illinois (or ability to obtain within 90 days)
- Minimum 3 years crisis intervention experience with vulnerable populations
- Advanced knowledge of trauma-informed care principles
- Experience with electronic health records (EHR) systems
- Valid driver's license with reliable transportation
- Bilingual English/Spanish proficiency preferred
- Certification in suicide prevention and de-escalation techniques
