Responsibilities
- Develop and implement comprehensive care plans for clients navigating housing instability, mental health challenges, and systemic barriers
- Lead crisis intervention teams providing immediate support during emergencies and trauma-informed care
- Collaborate with healthcare providers, educators, and government agencies to create integrated support networks
- Conduct thorough psychosocial assessments and maintain detailed documentation compliant with HIPAA regulations
- Design and facilitate community workshops on financial literacy, parenting skills, and mental wellness
- Mentor junior staff members and contribute to agency-wide program evaluation and improvement
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW (Licensed Clinical Social Worker) certification in Illinois
- Minimum 5 years of direct service experience in community-based social services
- Proven expertise in trauma-informed care and crisis intervention protocols
- Strong proficiency in electronic health record systems (EHR) and case management software
- Exceptional cultural competency with diverse urban populations
- Valid Illinois driver's license with reliable transportation for home visits
