Responsibilities
- Develop and implement individualized care plans for at-risk populations (youth, families, unhoused individuals)
- Lead crisis intervention and trauma-informed counseling sessions
- Coordinate multi-agency partnerships with healthcare, legal, and educational institutions
- Advocate for policy changes through community organizing and legislative outreach
- Conduct comprehensive psychosocial assessments and progress evaluations
- Train and mentor junior social work staff and volunteers
- Manage caseload documentation in compliance with HIPAA and agency standards
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois (or eligible within 90 days)
- 3+ years direct service experience in community-based social services
- Expertise in crisis intervention and trauma-informed care models
- Proven success in collaborative community partnerships and advocacy
- Strong cultural competency serving diverse urban populations
- Advanced knowledge of local social service systems and resources
- Certification in Mental Health First Aid preferred
