Responsibilities
- Develop and implement individualized care plans for at-risk populations
- Conduct comprehensive psychosocial assessments and crisis interventions

- Advocate for policy changes addressing systemic social inequities
- Provide trauma-informed counseling to individuals and families
- Lead outreach programs for underserved communities
- Maintain detailed case documentation and compliance records
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LSW licensure in Illinois
- 3+ years direct practice with vulnerable populations
- Experience in crisis intervention and trauma-informed care
- Strong advocacy and community organizing skills
- Bilingual proficiency in Spanish or Polish preferred
- Valid Illinois driver's license with reliable transportation
