Responsibilities
- Develop and implement comprehensive care plans for at-risk populations including survivors of domestic violence and individuals experiencing homelessness
- Conduct psychosocial assessments and provide trauma-informed crisis intervention services
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and government agencies
- Facilitate therapeutic groups focusing on life skills, mental health resilience, and community integration
- Advocate for clients through policy navigation, benefit enrollment assistance, and legal support coordination
- Maintain detailed case documentation ensuring compliance with HIPAA and agency standards
- Lead community outreach initiatives to identify service gaps and expand program accessibility
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as LCSW in New York State (or eligibility within 6 months)
- Minimum 5 years direct clinical experience with trauma-informed populations
- Advanced training in crisis intervention, motivational interviewing, and cultural humility
- Proven success in securing housing, employment, and healthcare resources for vulnerable clients
- Exceptional documentation skills with proficiency in electronic health record systems
- Demonstrated experience leading community coalitions or policy advocacy projects
