Responsibilities
- Develop and implement comprehensive care plans for at-risk children and families
- Conduct thorough psychosocial assessments and risk evaluations
- Coordinate multidisciplinary teams including therapists, educators, and medical professionals
- Provide crisis intervention and trauma-informed counseling services
- Advocate for clients within legal and healthcare systems
- Maintain accurate case documentation and compliance with state regulations
- Mentor junior social workers and supervise practicum students
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- Minimum 3 years' experience in child welfare or family services
- LCSW or LMSW licensure in New York State
- Proficiency in trauma-informed care and crisis intervention models
- Demonstrated cultural competency with diverse urban populations
- Strong case management and documentation skills
- Valid NYS driver's license with clean record
- Experience with electronic health records (EHR) systems
