Responsibilities
- Provide direct counseling and case management for at-risk youth and families
- Develop and implement individualized service plans aligned with client goals
- Collaborate with schools, courts, and healthcare providers to coordinate care
- Conduct comprehensive psychosocial assessments and risk evaluations
- Lead crisis intervention protocols for emergency situations
- Maintain detailed case documentation and comply with regulatory standards
- Mentor junior staff and facilitate community workshops
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- Licensure as LCSW in Illinois (or ability to obtain within 90 days)
- 3+ years experience in child welfare or family services
- Strong knowledge of trauma-informed care principles
- Experience with electronic health records (EHR) systems
- Excellent crisis intervention and de-escalation skills
- Bilingual proficiency in Spanish preferred
- Valid Illinois driver's license with clean record
