Responsibilities
- Assess and develop personalized care plans for at-risk children and families
- Provide crisis intervention and therapeutic counseling using evidence-based modalities
- Collaborate with schools, courts, and healthcare providers for coordinated care
- Conduct home visits and maintain comprehensive case documentation
- Lead community outreach programs to strengthen family support networks
- Mentor social work interns and supervise case management staff
- Analyze community needs to inform program development and policy advocacy
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois with 3+ years post-licensure experience
- Expertise in child welfare, family systems, and trauma-informed care
- Strong crisis intervention and conflict resolution skills
- Proficiency in electronic health records (EHR) systems
- Experience with program evaluation and data-driven reporting
- Valid Illinois driver's license and reliable transportation
- Bilingual (English/Spanish) preferred
