Our Chicago-based organization has been empowering communities for over 25 years through innovative social service programs. We're seeking a dedicated individual to lead our case management initiatives and advocate for systemic change.
Responsibilities
- Develop and implement comprehensive case management plans for individuals and families facing complex challenges
- Provide crisis intervention and trauma-informed counseling to at-risk populations
- Collaborate with community partners to coordinate resources and support services
- Advocate for client needs within healthcare, legal, and educational systems
- Conduct thorough assessments and maintain accurate documentation
- Mentor junior staff and supervise practicum students
- Participate in program evaluation and continuous improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in the state of Illinois
- Minimum 5 years of direct practice experience in social services
- Proven expertise in crisis intervention and trauma-informed care
- Strong cultural competency and ability to work with diverse populations
- Excellent communication and advocacy skills
- Experience in program development and evaluation
- Valid driver's license and reliable transportation
