Responsibilities
- Develop and implement comprehensive case management plans for clients facing homelessness, domestic violence, and mental health challenges
- Conduct psychosocial assessments and crisis interventions in high-pressure situations
- Collaborate with multidisciplinary teams to coordinate wraparound services including housing, healthcare, and legal aid
- Advocate for policy changes at local government level to address systemic barriers
- Train and mentor junior social workers on trauma-informed care methodologies
- Maintain meticulous documentation while ensuring HIPAA compliance
Qualifications
- Master's degree in Social Work (MSW) with NY State LMSW licensure
- Minimum 5 years' experience in direct client services within urban social services
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Proven success in grant writing and program development for federal/state funding
- Advanced proficiency in electronic health records (EHR) systems
- Demonstrated cultural competency with diverse populations including LGBTQ+ and immigrant communities
- Valid driver's license with reliable transportation
