Responsibilities
- Develop and implement individualized case management plans for clients facing housing instability, mental health challenges, and economic hardship
- Coordinate multi-disciplinary interventions with healthcare providers, legal advocates, and community organizations
- Conduct comprehensive psychosocial assessments and crisis interventions in high-stakes environments
- Lead trauma-informed support groups for survivors of domestic violence and substance abuse
- Advocate for policy changes by collaborating with city officials and non-profit coalitions
- Maintain meticulous documentation for compliance with federal funding requirements
- Mentor junior staff through clinical supervision and peer review sessions
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- Licensure as LCSW in Illinois with minimum 3 years post-MSW experience
- Expertise in crisis intervention and trauma-informed care methodologies
- Proven success in securing housing vouchers and SNAP benefits for vulnerable populations
- Experience with electronic health records (EHR) systems like Epic or Cerner
- Bilingual proficiency in Spanish preferred
- Valid Illinois driver's license with clean driving record
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
