Responsibilities
- Provide clinical assessments and develop individualized care plans for at-risk populations
- Lead crisis intervention services and coordinate multi-disciplinary support teams
- Facilitate therapeutic group sessions and family counseling sessions
- Conduct community outreach and program evaluations to identify service gaps
- Maintain accurate case documentation while ensuring compliance with HIPAA regulations
- Collaborate with healthcare providers, schools, and government agencies
- Mentor junior staff and contribute to agency training initiatives
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LSW licensure in Illinois with 3+ years post-graduate experience
- Proven expertise in crisis intervention and trauma-informed care
- Experience with electronic health records (EHR) systems
- Bilingual proficiency in Spanish preferred
- Strong analytical skills for program evaluation and data-driven decision making
- Valid Illinois driver's license with reliable transportation
- Understanding of Chicago's social service landscape and resources
