Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized service plans
- Provide crisis intervention, counseling, and case management for at-risk populations
- Collaborate with community partners to expand access to housing, healthcare, and employment resources
- Advocate for client needs through policy engagement and systems navigation
- Train and mentor junior social workers on trauma-informed care practices
- Monitor outcomes and document case progress using electronic health records
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois with 3+ years post-MSW experience
- Expertise in crisis intervention, housing instability, and mental health support
- Experience developing trauma-informed care programs
- Strong cultural competency with diverse urban populations
- Advanced knowledge of local social service systems and resources
- Valid driver's license and reliable transportation
