Responsibilities
- Develop and implement comprehensive care plans for individuals and families facing complex psychosocial challenges
- Provide trauma-informed counseling and crisis intervention services to at-risk populations
- Coordinate interdisciplinary case management across healthcare, housing, and legal systems
- Conduct community outreach programs to identify service gaps and mobilize resources
- Lead advocacy initiatives for policy reform affecting vulnerable communities
- Maintain accurate documentation while ensuring HIPAA compliance and ethical standards
- Mentor junior staff through clinical supervision and skill-building workshops
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in the State of Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care methodologies
- Familiarity with Chicago's social services ecosystem and community resources
- Advanced knowledge of Medicaid/Medicare reimbursement systems and insurance navigation
- Strong cultural competency in working with diverse populations including LGBTQIA+ and immigrant communities
- Excellent written communication skills for grant writing and program evaluation
- Valid Illinois driver's license and reliable transportation for home visits
