Responsibilities
- Conduct comprehensive psychosocial assessments and develop personalized care plans for clients facing housing instability, mental health challenges, and economic hardship
- Lead crisis intervention protocols and provide trauma-informed counseling to individuals and families
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and community leaders
- Design and facilitate group workshops on financial literacy, parenting skills, and mental wellness
- Advocate for systemic change by participating in policy initiatives and community coalitions
- Maintain accurate documentation while ensuring HIPAA compliance and ethical record-keeping
- Mentor junior staff and supervise fieldwork students pursuing social work degrees
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- Licensed Clinical Social Worker (LCSW) in the state of Illinois
- Minimum 5 years of direct practice experience in community-based social services
- Proven expertise in crisis intervention and trauma-informed care methodologies
- Strong knowledge of Chicago's social service landscape and resource networks
- Experience developing and implementing evidence-based community programs
- Excellent cultural competency with diverse urban populations
- Advanced proficiency in EHR systems and data-driven reporting
