Responsibilities
- Develop and implement comprehensive care plans for vulnerable populations including families, veterans, and unhoused individuals
- Conduct psychosocial assessments and crisis interventions in high-pressure community settings
- Collaborate with multidisciplinary teams (medical, legal, education) to coordinate wraparound services
- Advocate for systemic policy changes through data-driven reporting and community partnerships
- Train and mentor junior staff on trauma-informed care methodologies
- Manage caseloads of 25+ clients while maintaining 95% documentation compliance
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois with 3+ years post-MSW experience
- Proven success in navigating complex community resources (housing, healthcare, legal aid)
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
- Experience with electronic health records (Epic or Cerner)
- Spanish fluency required for bilingual client population
- Valid Illinois driver's license and reliable transportation
