Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized intervention plans
- Provide crisis counseling and trauma-informed support to high-risk populations
- Coordinate care with healthcare providers, schools, and community organizations
- Advocate for clients through systems navigation and legal proceedings
- Lead case management teams and mentor junior social workers
- Design and implement outreach programs for underserved communities
- Maintain detailed documentation ensuring compliance with HIPAA and agency standards
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LSW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care
- Experience developing community-based programs and resource networks
- Strong knowledge of Medicaid, SNAP, and disability benefit systems
- Exceptional communication skills with diverse populations
- Ability to manage complex caseloads and prioritize competing demands
- Cultural competency working with LGBTQ+, immigrant, and unhoused populations
