Responsibilities
- Develop and implement individualized treatment plans for vulnerable populations including youth, veterans, and families experiencing homelessness
- Conduct comprehensive psychosocial assessments and crisis interventions in high-stakes community settings
- Collaborate with multidisciplinary teams to coordinate wraparound services and resource navigation
- Lead community outreach programs focused on mental health awareness and prevention strategies
- Mentor and train junior social workers through supervision and professional development sessions
- Advocate for systemic policy changes affecting social service delivery at municipal level
- Maintain meticulous documentation compliant with HIPAA and state regulations
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois with minimum 3 years post-licensure experience
- Proven expertise in trauma-informed care and cognitive behavioral therapy modalities
- Demonstrated success in grant writing and program evaluation for social services
- Experience supervising multidisciplinary teams of 5+ professionals
- Advanced knowledge of Chicago's social service landscape and community resources
- Certification in Crisis Intervention or Trauma-Focused CBT preferred
- Valid Illinois driver's license with reliable transportation
