Responsibilities
- Develop and implement individualized care plans for clients facing complex psychosocial challenges
- Conduct comprehensive assessments and risk evaluations using evidence-based frameworks
- Coordinate interdisciplinary case management with healthcare providers, schools, and legal services
- Provide crisis intervention and trauma-informed counseling to diverse populations
- Lead community outreach initiatives to identify and address systemic barriers to care
- Maintain accurate documentation and compliance with HIPAA and state regulations
- Mentor junior staff and contribute to program development and evaluation
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as LCSW in Illinois (or willingness to obtain within 6 months)
- Minimum 5 years of direct service experience in clinical social work
- Expertise in crisis intervention and trauma-informed care methodologies
- Proven ability to manage complex caseloads with diverse populations
- Familiarity with Chicago's social services ecosystem and resources
- Strong analytical skills for data-driven program improvement
- Excellent written and verbal communication with stakeholders
