Responsibilities
- Develop and implement personalized intervention plans for clients facing complex social, emotional, and economic challenges
- Conduct comprehensive assessments to identify needs, risks, and barriers to well-being
- Provide crisis intervention, counseling, and advocacy services to vulnerable populations
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal professionals
- Maintain accurate case documentation and ensure compliance with regulatory standards
- Facilitate support groups and community workshops on mental health and resilience
- Contribute to program development and policy recommendations for service improvement
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 3 years of direct experience in clinical or community social services
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Proven expertise in trauma-informed care and cultural competency practices
- Strong assessment, crisis intervention, and case management skills
- Excellent communication abilities with diverse populations and stakeholders
- Valid Ontario driver's license and reliable transportation for home visits
- Knowledge of local community resources and social service systems
