Responsibilities
- Provide crisis intervention and ongoing counseling to vulnerable populations including youth, families, and unhoused individuals
- Develop and implement individualized care plans with measurable outcomes
- Coordinate multi-agency support networks including healthcare, housing, and legal services
- Conduct comprehensive psychosocial assessments and risk evaluations
- Advocate for systemic policy changes through community partnerships
- Maintain detailed case documentation compliant with state regulations
- Lead trauma-informed support groups for diverse demographic groups
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- NYS LMSW license with 3+ years post-licensure experience
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Proven experience in crisis intervention and case management
- Strong understanding of NYC social services ecosystem
- Bilingual proficiency in Spanish or Mandarin required
- Advanced knowledge of HIPAA and confidentiality protocols
- Demonstrated success in grant reporting and outcome measurement
