Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized intervention plans
- Provide crisis intervention and trauma-informed counseling to individuals and families
- Facilitate connections to community resources including housing, healthcare, and employment services
- Lead outreach programs targeting vulnerable populations across Chicago neighborhoods
- Maintain accurate case documentation and comply with HIPAA regulations
- Train and mentor junior social workers on best practices and ethical standards
- Analyze community needs data to inform program development and advocacy efforts
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Valid Illinois LSW/LCSW license required
- Minimum 5 years of direct service experience in social services
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
- Proven success in navigating complex community resource networks
- Advanced proficiency in electronic health records (EHR) systems
- Bi-lingual in Spanish or Mandarin strongly preferred
- Demonstrated commitment to cultural humility and anti-racist practices
