Responsibilities
- Develop and implement individualized care plans for clients facing complex social challenges
- Provide crisis intervention, counseling, and advocacy for vulnerable populations
- Collaborate with multidisciplinary teams including healthcare providers and law enforcement
- Conduct thorough assessments and maintain accurate client documentation
- Design and lead community workshops on mental health awareness and resilience
- Research and secure funding/resources for underserved communities
- Mentor junior social workers and contribute to program innovation
Qualifications
- Master's degree in Social Work (MSW) with registered clinical designation
- Minimum 5 years experience in direct client services
- Certification in trauma-informed care and crisis intervention
- Proven expertise in case management and resource navigation
- Strong understanding of Canadian social service systems and policies
- Excellent communication skills with diverse populations
- Valid driver's license and reliable transportation
- Bilingual (English/French) preferred
