Responsibilities
- Develop and implement comprehensive care plans for 25+ high-risk families facing housing instability and mental health challenges
- Coordinate multi-agency partnerships with healthcare providers, legal aid, and educational institutions
- Conduct biopsychosocial assessments using evidence-based frameworks to identify service gaps
- Provide crisis intervention and trauma-informed support during emergency situations
- Train junior staff on cultural humility practices and ethical documentation standards
- Analyze program outcomes to refine service delivery models using data-driven approaches
- Advocate for policy changes at community board meetings and legislative hearings
Qualifications
- Master's degree in Social Work (MSW) with NY State LMSW licensure required
- 5+ years progressive experience in direct service case management with diverse populations
- Advanced certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Proficiency with electronic health records (EHR) systems and data visualization tools
- Demonstrated success in securing housing vouchers and SNAP benefits for vulnerable households
- Bilingual proficiency in Spanish and English preferred
- Experience supervising social work interns and mentoring junior staff
- Valid driver's license with clean record for home visits
