Responsibilities
- Develop and implement personalized case management plans for vulnerable populations including families, veterans, and at-risk youth
- Conduct comprehensive psychosocial assessments and provide crisis intervention services in high-stakes situations
- Collaborate with healthcare providers, schools, and government agencies to coordinate holistic support systems
- Facilitate therapeutic group sessions and workshops focused on resilience-building and skill development
- Advocate for client needs within legal and institutional frameworks, ensuring equitable access to resources
- Maintain meticulous documentation while adhering to HIPAA and confidentiality protocols
- Mentor junior staff and contribute to program evaluation and continuous improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution required
- LCSW (Licensed Clinical Social Worker) certification in Illinois with at least 3 years post-licensure experience
- Proven expertise in trauma-informed care and crisis intervention methodologies
- Demonstrated success in collaborating with diverse stakeholders including healthcare, education, and justice systems
- Advanced knowledge of community resources and public assistance programs in metropolitan areas
- Strong analytical skills for complex case formulation and evidence-based intervention design
- Excellent written and verbal communication with ability to navigate sensitive conversations
- Cultural humility and experience working with marginalized populations
