Responsibilities
- Develop and implement comprehensive care plans for individuals and families facing complex socio-economic challenges
- Lead crisis intervention protocols for clients experiencing homelessness, domestic violence, or mental health emergencies
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal advocates
- Conduct psychosocial assessments and provide evidence-based therapeutic interventions
- Oversee case management for 25+ high-risk clients, ensuring continuity of care
- Advocate for systemic policy changes through community outreach and legislative engagement
- Maintain meticulous documentation compliant with HIPAA and state regulations
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW/LICSW license in good standing with Massachusetts Board of Registration
- Minimum 5 years clinical experience with vulnerable populations
- Advanced training in trauma-informed care and crisis intervention models
- Proven ability to navigate complex community resources and entitlement programs
- Exceptional cultural competency with diverse urban populations
- Strong analytical skills for program evaluation and outcome measurement
- Valid Massachusetts driver's license and reliable transportation
