Responsibilities
- Develop and implement comprehensive case management plans for vulnerable individuals and families
- Provide crisis intervention, counseling, and advocacy services to community members
- Collaborate with healthcare providers, schools, and government agencies to coordinate resources
- Conduct needs assessments and develop evidence-based intervention strategies
- Lead community outreach programs and workshops on mental health and social justice
- Maintain accurate case documentation and comply with regulatory requirements
- Mentor junior staff and supervise social work interns
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as a Licensed Clinical Social Worker (LCSW) in Illinois
- Minimum 5 years of direct experience in crisis intervention and case management
- Proven ability to work with diverse populations including refugees, LGBTQ+ individuals, and trauma survivors
- Strong knowledge of community resources, healthcare systems, and social welfare policies
- Excellent communication skills with ability to navigate complex systems
- Certification in Trauma-Focused Cognitive Behavioral Therapy preferred
