Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health challenges, and systemic barriers
- Lead crisis intervention protocols and provide trauma-informed counseling to at-risk populations
- Coordinate multidisciplinary services including healthcare, legal aid, and employment support
- Conduct comprehensive psychosocial assessments and maintain accurate documentation
- Advocate for policy changes and community resources through partnerships with local organizations
- Mentor junior staff and supervise fieldwork placements for social work interns
- Analyze program outcomes and contribute to continuous improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- LCSW or LMSW license in New York State
- Minimum 5 years' experience in direct social services with vulnerable populations
- Expertise in trauma-informed care and crisis intervention techniques
- Proven ability to navigate complex community systems and resources
- Strong cultural competency and bilingual Spanish/English proficiency
- Experience with electronic health records (EHR) and data-driven assessment tools
- Valid NYS driver's license and reliable transportation
