Responsibilities
- Develop and implement individualized care plans for vulnerable populations
- Conduct psychosocial assessments and crisis interventions
- Coordinate with healthcare providers, schools, and government agencies
- Lead community outreach programs and workshops
- Maintain accurate client records and documentation
- Supervise junior social work staff and interns
- Advocate for policy changes affecting client communities
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- 3+ years direct social services experience
- LCSW or LMSW license in New York State
- Bilingual proficiency in English/Spanish preferred
- Experience with trauma-informed care practices
- Strong crisis intervention and de-escalation skills
- Knowledge of NYC social services systems
- Valid NY driver's license
