Responsibilities
- Develop and implement comprehensive care plans for clients facing complex psychosocial challenges
- Conduct biopsychosocial assessments and crisis interventions in diverse community settings
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal advocates
- Supervise 3-5 junior social workers through clinical oversight and professional development
- Lead community outreach initiatives to identify at-risk populations and reduce service barriers
- Maintain accurate case documentation compliant with state and federal regulations
- Analyze program data to inform service improvements and grant reporting
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Expertise in trauma-informed care and crisis management protocols
- Proven ability to supervise social work interns and new professionals
- Familiarity with electronic health records (EHR) systems, particularly Epic
- Certification in motivational interviewing or DBT preferred
- Experience with program evaluation and outcome measurement
- Valid Illinois driver's license and reliable transportation
