Responsibilities
- Provide intensive case management and crisis intervention for at-risk families
- Develop and implement personalized care plans addressing complex psychosocial needs
- Coordinate multidisciplinary teams including healthcare providers, educators, and legal advocates
- Lead community outreach programs and resource navigation workshops
- Maintain detailed client documentation while ensuring HIPAA compliance
- Conduct psycho-social assessments and facilitate therapeutic support groups
- Advocate for policy changes affecting vulnerable populations
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Proven expertise in trauma-informed care and crisis intervention protocols
- Strong assessment skills with diverse populations including refugees and LGBTQ+ communities
- Experience developing community partnerships and resource networks
- Proficiency in electronic health records (EHR) systems
- Valid Illinois driver's license and reliable transportation
