Responsibilities
- Lead comprehensive assessments and develop individualized care plans for vulnerable populations
- Coordinate transitional housing programs and navigate complex social service systems
- Provide crisis intervention and long-term support for clients facing domestic violence or homelessness
- Train and supervise 3-5 junior social workers using evidence-based methodologies
- Collaborate with healthcare providers, schools, and government agencies
- Design and implement community workshops on mental health and resource accessibility
- Maintain detailed case documentation while ensuring HIPAA compliance
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- 5+ years of direct practice in mental health or housing services
- LCSW or LMSW license in New York State
- Certification in Trauma-Focused CBT or DBT
- Experience with electronic health records (Epic or Cerner preferred)
- Bilingual proficiency in Spanish or Mandarin required
- Proven ability to manage high caseloads with competing priorities
- Valid NYS driver's license with clean record
