Responsibilities
- Develop and implement individualized care plans for clients experiencing homelessness, domestic violence, or mental health challenges
- Collaborate with interdisciplinary teams to coordinate comprehensive support services
- Conduct risk assessments and provide immediate crisis intervention for high-need cases
- Advocate for clients within legal, healthcare, and educational systems
- Lead community workshops on trauma-informed care and resource navigation
- Maintain detailed case documentation and ensure compliance with HIPAA regulations
- Mentor junior staff and supervise social work interns
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Proven expertise in crisis intervention and trauma-informed practices
- Experience working with diverse populations including LGBTQ+ and refugee communities
- Strong knowledge of Chicago's social services landscape and resource networks
- Excellent communication skills with ability to navigate complex systems
- Valid Illinois driver's license and reliable transportation
