Responsibilities
- Develop and implement individualized care plans for vulnerable populations including at-risk youth and families
- Coordinate multidisciplinary teams to address complex social determinants of health
- Conduct comprehensive psychosocial assessments and crisis interventions
- Lead community outreach programs and advocacy initiatives
- Maintain accurate documentation and ensure compliance with HIPAA regulations
- Mentor junior staff and supervise practicum students
- Analyze program outcomes and recommend evidence-based improvements
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Proven success in case management and crisis intervention
- Familiarity with trauma-informed care and cultural competency frameworks
- Experience with electronic health records (EHR) systems
- Strong analytical skills with ability to interpret complex social data
- Valid driver's license and reliable transportation
- Bilingual (English/Spanish) preferred
