Responsibilities
- Develop and implement personalized intervention plans for vulnerable populations including youth, families, and homeless individuals
- Conduct comprehensive psychosocial assessments and crisis interventions in emergency settings
- Collaborate with multidisciplinary teams including healthcare providers, law enforcement, and educators
- Oversee caseload management while maintaining documentation compliance with state/federal regulations
- Lead community workshops on mental health awareness and resource navigation
- Advocate for policy changes affecting social service delivery systems
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LMSW license in New York State
- Minimum 5 years experience in direct practice with diverse populations
- Advanced training in trauma-informed care and crisis intervention
- Proficiency in electronic health records (EHR) systems
- Bilingual English/Spanish proficiency preferred
- Valid NY driver's license with clean record
