Responsibilities
- Develop and implement comprehensive care plans for high-risk clients, coordinating with healthcare providers, legal advocates, and community organizations
- Conduct thorough psychosocial assessments and crisis interventions, ensuring immediate safety and long-term stability
- Lead trauma-informed therapy sessions utilizing evidence-based modalities (CBT, DBT, Motivational Interviewing)
- Design and execute outreach programs targeting at-risk youth and families in underserved boroughs
- Collaborate with policy advisors to draft recommendations for systemic improvements in social service delivery
- Supervise 3-5 junior social workers through mentorship, case reviews, and professional development planning
- Maintain meticulous documentation compliant with HIPAA and state regulations while ensuring client confidentiality
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution with clinical specialization
- LCSW or LMSW licensure in New York State with 5+ years post-licensure experience
- Proven track record in crisis intervention and trauma-informed care with vulnerable populations
- Advanced proficiency in electronic health records (EHR) systems and data-driven case management
- Experience developing grant proposals and securing funding for community initiatives
- Strong understanding of NYC's social service ecosystem, including ACS, DFTA, and community-based organizations
- Bilingual proficiency in Spanish or Mandarin preferred
- Excellent conflict resolution skills with demonstrated ability to navigate complex family dynamics
